I hereby declare as follows:
I do not have any symptoms of COVID-19 in the past 14 days nor have I been diagnosed or treated for the same within the past 60 days.
During the Sadhana, if I experience any symptoms of ill health, I shall immediately inform the teacher and adhere to the protocols.
I understand that, due to the current COVID-19 Pandemic situation, Yoga Systems has formulated certain protocols and guidelines to prevent the spread of COVID and ensure the health and well-being of the participants in the yoga program. I hereby agree to follow all the protocols (wearing a mask, maintaining minimum of 1 meter of social distancing, etc.), and guidelines that are already in place and that will be formulated from time to time during the duration of the program. The guidelines may change in line with the guidelines and recommendations notified by the Government from time to time.
I agree to provide full and correct details of all/any of my medical conditions. In the event of any emergency or unforeseen health-related complications, I agree to abide by the recommendations made in my best interests by the teacher and volunteers. Any medical expenses incurred in this regard will be fully borne by me.
I agree to indemnify and hold the program teacher/volunteers harmless with respect to my health, ailments (including Covid-19 ), or recovery during my participation in the program. They shall not be liable and do not hold any responsibility for the same.
I agree that the program organizers shall have all rights to unilaterally terminate my participation in the program should I fail to scrupulously follow any of the protocols/ guidelines already issued/to be issued by them or Government authorities.
I hereby willingly undertake to attend this program completely. I take full responsibility for the result and indemnify the organizers against all claims and suits. I will not communicate the contents of the program, either directly or indirectly to anyone else.
I acknowledge and affirm that I have read, understood and made the above declaration and will abide by it at all times during my participation in this program.
I also agree that in the event of incomplete or false information given by me, I will not be allowed to participate in the program.
